Community Health Visitor (Contract, Div of Med-KIDS 0-3)

The Community Health Visitor (CHV) will deliver integrated preventive health and social care service provision and coordination for mother and child, focusing on antenatal and postnatal care, developmental-centered parenting, parent child interaction and family well-being, with the aim of optimising every child's potential for healthy growth and development. To be the main point-of-service-coordination between patient/client/family with healthcare and social service systems.

Job requirements:

• Degree in Nursing (preferred), Early Childhood, Psychology or
• Specialist Diploma (Early Childhood/Psychology/Social Work) with 5 years relevant experience
• Relevant Experience of working with vulnerable children and families in a community setting and/or with infant and child development work
• Scope of work is physically and emotionally demanding
• Includes travelling to and from Client's homes for service delivery and emotionally intense work with vulnerable families, children and young infant
• Dynamic, problem-solver, efficient, resourceful and independent individual
• Demonstrate a high level of initiative and organisational skills
• Strong interpersonal, communication and writing skills are essential
