Staff Nurse

You will assist patients with their continuing care needs, to develop a safe, smooth, and timely discharge care plan from the hospital to the community. This will include actively participating in the daily rounds and discharge huddles with the physicians and care teams to facilitate communication and planning with the patients as well as their family members and caregivers.

You are expected to identify and manage potential issues to the discharge plans, and collaborate with the multidisciplinary care teams to expedite diagnostic testing, treatments and consultations. Additionally, you will monitor and follow through with the patients’ post-discharge through regular follow-up calls, and referrals to appropriate community and social agencies, according to the care needs of the patients and their caregivers. You will be required to support and work collaboratively with all stakeholders for the patients’ seamless transition to step-down care.

Requirements:

• Registered Nurse or Nursing qualification recognised by the Singapore Nursing Board with at least 1-2 years’ nursing experience in an acute hospital setting

• Proficient in Microsoft Office applications with database knowledge

• Team player with good interpersonal and communication skills

• Possess good analytical and organisational skills

• Able to work independently as well as in a team
