Executive, Right Siting and Share Care (PHIC)

Job Responsibilities

The Executive is responsible for developing and implementing shared care initiatives within the organization. The Executive will coordinate care between primary care providers, specialists, and other healthcare professionals to ensure integrated and person-centered care.

The duties & responsibilities of the Executive include but is not limited to the following:

• Design, develop and implement right siting and shared care programmes to enhance the coordination and efficiency of patient care.
• Establish protocols and procedures to ensure effective integration of right siting initiatives and shared care practices.
• Serve as the primary point of contact for right siting and shared care initiatives, facilitating communication between primary care providers, specialists, and other healthcare professionals.
• Monitor the quality and effectiveness of right siting and shared care programmes by analysing data.
• Oversee day-to-day operations and ensure that all activities efficient and effective in meeting the needs of patients and providers.

Job Requirements

• Degree or equivalent in healthcare administration with at least 4 years of working experience. (Senior Executive) Experience in healthcare management or care coordination would be an added advantage
• Good interpersonal skills to communicate constructively with colleagues and stakeholders
