Liver Transplant Coordinator
We are seeking a dedicated and compassionate Transplant Coordinator. You will be responsible for overall co-ordination and management of pre-and post-liver transplant patients from Liver/MultiVisceral/intestinal Transplant Programs.
Key Responsibilities

Coordination of the evaluation process for Liver/MultiVisceral/intestinal Transplant of living donation and transplantation, preparation of donors to undergo living donation, preparation of recipients for transplant surgery and the follow up of recipients and donors after surgery

Registration and management of patients on the National Waiting List for deceased-donor liver transplant

Reviewing the work processes in the follow up of the post-transplant patients in order to ensure that care for patients are rendered efficiently

Providing counselling to patients (including potential living donors) and their families on transplantation process

Educating patient regarding their primary physician’s recommendation and the reason to support the physician’s recommendation

Ordering and basic interpretation of diagnostic and laboratory tests

Ordering appropriate treatments and non-pharmacologic therapies according to hospital protocols

Providing post-transplant teaching and education to recipients and donors according to individual patient’s level of understanding

Assisting in ad hoc patients’ complaints and care arrangement based on critical thinking and communication with relevant stakeholders

Assisting in outpatient follow up of patients in Transplant Clinic alongside Transplant physician and pharmacist

Reinforcing the importance of pre/post-transplant follow-up

Working with the various transplant specialties in ensuring all clinical instructions are tracked, monitored and follow up

Collating the necessary data and to manage the various database systems that the program is utilizing

Working closely with stakeholders to ensure accurate collection and record of data

Collaborating with stakeholders to support defined clinical needs.

Assisting in various research activities involving liver transplantation

Participating in the organization and delivery of transplant education, patient support activities and community outreach events

Handling ad-hoc duties as assigned
Requirements
-
Degree in Nursing / Life Science / Psychology / Health Science
-
Willing to perform on-call duties
-
Experience in healthcare or other people‑centric roles
-
Strong communication, teamwork, and patient‑education skills
