Senior/ Executive (Clinical Events Data Management)

You will provide operational support in the management of Hospital's clinical events safety program through clinical and systemic reviews of the incidents. 
 

Your duties include: 

• Review, investigate and triage clinical incidents reported in the SingHealth Incident Reporting System (IRS)
• Provide technical, administrative as well as support the function of the IRS
• Monitor and analyze institution incident trends or indicators and provide visualization and insights to various stakeholders
• Facilitate discussion / collaborate with stakeholders to effect practice changes through recommendations / insights that may be derived from patient safety data trends and incident review findings
• Escalate incidents of significant patient safety lapses or adverse events with major ongoing threat to Senior Leadership
• Collaborate and engage with patient safety partners from cluster level, other Singhealth institutions, and SGH representatives on IRS issues and patient safety incidents if required
• Conduct and facilitate Root Cause Analysis meeting for Serious Reportable Events (SREs) and ensure timely submission of reports to MOH and further follow-up
• Participate and drive patient safety and quality improvement initiatives

Requirements

• Degree in Health Sciences, preferably in Nursing, Medical and Allied Health disciplines
• Experience in quality improvement activities will be an advantage
• Excellent communication and interpersonal skills
