(HU), SingHealth's signature preventive health movement, you will work directly with residents in the community, journeying alongside them as they take meaningful steps towards better health.

We are seeking a Senior Wellbeing Coordinator or Assistant Wellbeing Coordinator Lead to provide personalised follow-ups with residents, deliver health recommendations and nudges via phone calls, SMS, email, and support the operational functions that keep HU running smoothly.

 

What You Will Do:

Your core work involves promoting HealthUP! and assisting residents to sign up, generating the Health Action Plan Personalised for You (HAPPY) for each assigned HU member, and consult with them to develop holistic care plans that reflect their individual goals and motivations. You will contact members regularly to walk them through their “HAPPY” health plan, assist them in screening, booking of appointments , track their journey and drive conversations that encourage healthy lifestyle changes at key milestones.

 

You will screen members for health and social care needs by administering questionnaires covering physical activity, diet, vaccination status, and mental wellbeing, using approaches such as motivational interviewing to identify the most appropriate recommendations. Where needed, you will make referrals to internal SingHealth care teams or external community partners, and escalate cases when health or social red flags are observed.

 

As a health coach and resource person, you will encourage members to adhere to their “HAPPY” or Healthier SG (HSG) health plans, adopt healthier lifestyles, and connect with suitable community resources and programmes. You will also assist in data collection, contribute to reports and key performance indicators (KPIs), and may occasionally support the training of newly hired or junior Wellbeing Coordinators (WBCs).

 

Job Requirements:

  • Bachelor’s Degree with at least 2 years of relevant experience. Fresh graduates may be considered for the Senior Wellbeing Coordinator role.
  • Candidate with a Diploma with at least 6 years of relevant experience can apply. The job level will be determined based on the candidate’s relevant experience.
  • Operational experience in the health, social, and community care sector is preferred
  • Strong verbal and written communication skills
  • Personable, empathetic, and able to relate well to people from diverse backgrounds, with a proactive and collaborative disposition
  • Versatile in adopting a variety of coaching methods suited to different resident profiles
  • Willingness to perform weekend duty to support community outreach and events
