Care Coordinator (Contract)

You will improve the care of Holistic Approach Lowering and Tracking Chronic Kidney Disease (HALT-CKD) programme patients and retard the progression of chronic kidney disease by educating patients on their conditions and optimizing their care. You will coordinate the provision of patients’ care through working closely with various healthcare professionals.

Responsibilities:

• Create and actively maintain HALT-CKD registry
• Provide clinical counselling to patients
• Support clinical practices through active monitoring on patients’ laboratory results and medication doses
• Support clinic’s quality improvement efforts and participate in quality improvement projects
• Ensure KPI deliverables are met
• Support programme-related events and meetings
• Collate data, statistics and consolidate slides and reports as required

Requirement:

• Degree in Nursing / Life Sciences / Medical Sciences or equivalent
• At least 1 to 2 years of healthcare experience preferred. Entry level candidates are welcome to apply
• Independent, resourceful, pro-active, detail-oriented and responsible
• Good interpersonal and communication skills; ability to converse in local dialects is advantageous
• Proficient in Microsoft Office applications
• Team player
