Pharmacist (Singapore Prisons)

The Pharmacist will be working at Changi Medical Facility in Singapore Prisons. The Pharmacist will be part of the medical team to deliver medical services and care at the Facility.

Responsibilities:

• Assist to oversee day to day operation of the pharmacies located within Singapore Prisons as well as supervising the work of the Pharmacy Technicians
• Collaborate with doctors and other healthcare professionals to provide patient care e.g. via optimizing use of medication, promoting health and wellness
• Engage in case discussions with doctors and other healthcare professionals
• Assist in drafting clinical protocols for safe use of medications within the Facility, and enforcing them
• Daily reviews of doctors’ orders to ensure safe and appropriate use of medications
• Conduct medication reconciliations for patients during transition of care

Requirements:

• Degree in Pharmacy with min 6 years of experience
• Registered Pharmacist with Singapore Pharmacy Council
• Good interpersonal and communication skills
