Programme Coordinator, Population Health & Integrated Care

You will coordinate and navigate comprehensive care for patients enrolled in the SKH@Home programme. You will serve as the primary point of contact between patients, next of kin, and the clinical care team (both in-house and outsourced). You will take ownership of the entire patient journey, from recruitment and onboarding through to programme completion, ensuring smooth enrolment, ongoing coordination, administrative integrity, and data compliance across the care pathway.

Requirements:

• Diploma in Healthcare Management or equivalent
• Prior experience in healthcare, community health, or home-care programmes is advantageous
• Strong communication and interpersonal skills
• Excellent organisational skills with an eye for detail and ability to multi-task
• Basic IT competency to set up and troubleshoot virtual platforms and patient-facing devices
