Programme Coordinator, Transitional Care & Community Medicine

You will assist the department in delivering the Hospital’s population health mandate in the Transitional Care/Community Medicine and Telehealth settings. You will be required to support the virtual ward setup within patients’ home. This includes helping to facilitate patient onboarding, providing financial counseling and patient education as well as assisting the clinical team in program operations. In addition, you will be the point of contact for patients and their next of kin for queries on the program.

Requirements:

• Diploma with at least 4 years of relevant experience
• A self-motivated team player who is able to multi-task, work under pressure and within tight timelines
• Experience in healthcare operations will be advantageous
• Excellent verbal and written communication skills
• Proficient in Microsoft Office applications, particularly Microsoft PowerPoint and Excel
