Senior / Assistant Manager, Transitional Care & Community Medicine
You will play a key role in developing and implementing new models of care delivery to enable seamless patient journeys that enhance clinical outcomes and satisfaction. Leveraging hands-on data analytics, you will engage diverse stakeholders and translate care models into effective operational workflows that support the programme goals. Additionally, you will participate in digital health integration initiatives, lead quality improvement projects, and provide secretariat support, including budget planning and tracking.

Requirements:
- Degree with at least 2 years of relevant working experience, preferably in public healthcare, involving transformation or care model development
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Experience with LEAN and/or Design Thinking would be advantageous
