Senior / Associate Executive, Service Planning & Care Integration
You will provide administrative support for the department. This includes scheduling of meetings, preparing reports, managing inventory items, administering onboarding for new staff or interns as well as consolidating and submitting the department's annual work plan and budget. You will collaborate with patients’ family/caregiver(s), clinic/hospital/specialty care providers, and community resources under the Primary Care Network to provide support to chronically ill patients. Apart from providing lifestyle counselling, you will also promote the importance of preventive health like health screenings and vaccinations, to empower patients on their self-care journey. In addition, you will be a key participant in quality improvement projects and other primary care partnership initiatives.
Requirements
- Diploma in Healthcare Management or equivalent with at least 2 years of working experience
- Operational experience in healthcare, social or community care sectors is preferred
- Good verbal and written communication skills
- Able to work independently as well as in a team
- Versatile in adopting various coaching methods to suit different patient profiles
- Proficient in Microsoft Office applications
